Referrals and initial appointments are handled by Mrs. Lois Evans,
office manager. She will gather basic intake information and answer any
questions you may have about the clinic.
Should you wish to use your insurance for reimbursement, we will do our best to
help you learn about your health care plan. You should be aware that most
health insurance plans limit their coverage for mental health, however. Managed
care plans require that you see a clinician from their local network and often
exclude procedures and diagnostic categories from their benefit packages. We
encourage you to become familiar with the benefits and exclusions of your
particular plan, and to contact your insurance carrier's claims representative
for questions about coverage.
Children and adolescents have got to be properly informed about the reasons for
their referral and nature of the clinical appointment in advance.
On the first visit the doctor will review old records and history, meet with
the patient and/or family for a psychiatric diagnostic examination, and make
recommendations for further evaluation and treatment.
Please complete the registration and medical forms ( Forms), and review HIPAA Practices ( Privacy) and office policies ( Office policies) prior to the appointment. Bring this information with you to facilitate the registration; otherwise, arrive 20 minutes
ahead of your scheduled appointment to complete forms.
Insurance co-payments, deductibles and
non-covered expenses are due on the day of your appointment. We accept cash, personal checks, Visa and MasterCard.
