APPOINTMENTS, "NO SHOW" AND LATE
Patients 18 YO and older are expected to
handle their own appointments.
Cancellations must be made by the patient or custodian 24 hours in advance
to avoid a charge. Leave message in the main voice mailbox, not the doctor's mailbox. We can't accept cancellations made by relatives or non-custodians. If you fail to show or cancel the same day of your
appointment you will be charged 100% of the customary fee. If you cancel
the day prior but less than 24 hours before your scheduled time you will be
charged 50% of the customary fee. The charge will be waived if the appointment
time can be assigned to another patient. Insurance companies will not reimburse
patients for charges concerning late cancellations or missed visits.
Call promptly if you need to reschedule an appointment. Set appointments can be rescheduled a maximum of two times.
After two consecutive no-shows or three
cancellations, the patient may be discharged from the clinic.
If you have a life-threatening medical
emergency and you are unable to immediately contact your doctor or any of the
office staff, please goto the nearestemergency room.
Telephone calls between appointmentsare
reserved for pressing medical/psychiatric issues. Calls are answered 24-hours a
day, seven days a week. Your doctor can be reached by calling our office
number. If your doctor is unavailable please leave a message with the
secretary or answering service. Your call shall be returned as soon as possible.
You may request your doctor to contact you
after business hours. Please make certain that your phone is
accessible toprivate callers -those who do not identify themselves
with the "Caller I.D." system-.
Many problems may be dealt with by advice over
the telephone and do not necessarily require an office visit. Your doctor
will be available during and after office hours to speak with you. Please
note that there may be a fee for consults or calls lasting 5-10 minutes or
longer, and that most insurance companies do not reimburse for this service.

very important that you follow the doctor's orders related to medications. You
or your pharmacist must call the office for refill authorization between
All requests for medication refills will be handled within 48 hours during
normal business hours. Leave message in the prescription refill voice mailbox, not the doctor's mailbox. Please remember torequest refills one
week prior to taking the last of the medication. Some controlled
medications require new written prescriptions from your doctor and can not be
processed over the telephone or fax. Request for prescriptions can not be
processed on weekends or holidays.
Keep all prescriptions of controlled
medications (e.g., Ritalin, Adderall, Dexedrine, Ativan, Xanax, Klonopin) in a
safe place. Lost or misplaced prescriptions will not be refilled. All
prescriptions are contingent on attendance to follow-up appointments. Any
tampering or alteration of prescriptions will result in immediate discharge of
the clinic. Stolen controlled medication will only be replaced after receipt of
a police report.
Payment is due in full at the time of service unless you are
covered by one of our contracted insurance carriers. Insurance co-payments,
deductibles and non-covered expenses are due on the day of the appointment.
Your billing receipt can be submitted to your insurance company for
reimbursement directly to you. The specific amount your insurance company
will reimburse varies depending upon your policy. Check with your
insurance company to determine the nature of your coverage.
Bills for services are issued on the 1st of each month and are
due in full within ten days. If your family is in treatment, the bill will
come in the name of one designated family member. Charges for evaluation and/or therapy
sessions, telephone consultation, record review, and preparation of reports will be
indicated on your bill. Telephone consultations, review of clinical records for diagnosis and treatment, and preparation of reports are charged at the hourly rate of $180, measured in ten minute intervals of $30.
All payments for services should be made directly to Psychiatry
Associates, P.C. There will be a 30 $ penalty charged for returned
checks. We honor payment with VISA and Master Card.
Any account 120 days in arrears will be assigned to a collection
agency or attorney. Failure to meet your financial obligations may result in
termination of clinical services.
