Referrals and initial appointments are handled by Mrs. Lois Evans, office manager. She will gather basic intake information and answer any questions you may have about the clinic.

Children and adolescents can only be examined with the authorization of their guardian or custodian. All youngsters have got to be properly informed about the reasons for their referral and nature of the clinical appointment in advance.

Complete the registration and medical forms ( Forms), and review HIPAA Practices ( Privacy) and office policies ( Office policies) prior to the appointment. Bring this information with you to facilitate the registration; otherwise, arrive 30 minutes ahead of your scheduled appointment to complete forms.

On the first visit the doctor will review old records and history, meet with the patient and/or family for a psychiatric diagnostic examination, and make recommendations for further evaluation and treatment.

If you wish to use your insurance for reimbursement, be aware that most health insurance plans limit their coverage for mental health services. We do not participate in Medicaid, Medicare or any managed health care plans, which require that you see a clinician from their local network. We encourage you to become familiar with the benefits and exclusions of your particular plan, and to contact your insurance carrier's claims representative for questions about coverage in advance. You are primarily responsible for payment of all services provided to you or your dependent minor.

Payment is due in full the day of your appointment. We accept cash, personal checks, Discover, Visa and MasterCard.
